Work Schedule:
Typically Monday to Friday from 9am to 5pm, with occasional evening shifts, but no working weekends

QUALIFICATIONS:
  • Must have a Bachelor's degree in social work or a related field. CWEL certification is a plus. Strong ability to engage with families. Proficiency in computer usage for necessary documentation. Previous experience with children with special needs and knowledge in Reunification, Adoption, and Sub-Guardianship is preferred.
  • Requirement of a valid Illinois Driver's License. Successful background check and passing of the State of Illinois exam on Licensing Standards for Foster Family Homes (402).

Position Overview:
Involves completing thorough adoption reports, sometimes spanning up to 20 pages.


Key Responsibilities:
  • Consulting on permanency matters, identifying and resolving barriers, and ensuring suitable placements for children.
  • Assisting in the utilization of community resources for parent home preparation.
  • Supporting post-adoption and Guardianship connections to the relevant departments before finalizing.
  • Securing additional service resources promptly for expediting permanency procedures.
  • Aiding in documentation completion for Adoption or Sub Guardianship subsidy initiation.
  • Managing submissions, monitoring, and following up on subsidy packet needs awaiting DCFS approval.
  • Facilitating worker support for out-of-state placements.
  • Establishing connections between caseworkers, recruitment specialists, adoptions staff, and the community to find suitable adoptive families.

DIRECT SERVICE
A. Providing transportation for clients as instructed by the Foster Care Supervisor

B. Collaborating with clients' foster care worker and the Foster Care Supervisor on all cases

D. Checking and responding to voicemails promptly.

E. Coordinating appointments or visits with parents and/or foster parents.

F. Contacting relevant parties for necessary transports or visits.


II. OTHER DUTIES
  • Active participation in YSBIV's Continuous Quality Improvement program
  • Regularly updating the outlook calendar and sharing it with the supervisor
  • Adherence to the established rules of the position
  • Submission of service tickets and visitation records to the supervisor weekly
  • Attendance to agency meetings, trainings, events, and supervisory meetings as guided by the Foster Care Supervisor

Salary:
$45,000.00 annually
