Social Service Coordinator - Part-Time Job Opening

We are currently seeking a Social Service Coordinator to join our team on a regular part-time basis, working 36 hours per week. This position involves splitting time between two senior residential communities, one with 36 units in Council Bluffs, IA, and the other with 20 units in Glenwood, IA.

JOB SUMMARY:

Assess and identify residents requiring services and coordinate service delivery.
Establish and maintain connections with community service providers.
Create and manage ongoing health and wellness programs for residents.
Develop a comprehensive list of community resources available to residents.
Act as a resident advocate and educate residents on self-advocacy.
Collaborate with community agencies and seek new services for residents.
Maintain positive relationships with residents to facilitate effective communication with service providers.
Work as a liaison to housing facility management, other departments, and external agencies offering social services and support for seniors.
Provide information and referrals for crisis intervention and conflict resolution.
Track resident referrals to agencies.
Keep accurate records, including daily and monthly reports.
Maintain resident files in compliance with HUD regulations.
Perform other assigned duties as required.

KNOWLEDGE & SKILLS:

Understanding of supportive services and community resources.
Ability to advocate for residents and solve problems.
Adherence to ethical standards and client confidentiality.
Effective collaboration with residents, staff, and partners.
Capacity to work independently with limited supervision.
Strong communication skills, both written and verbal.
Excellent time management abilities.
Proficiency in Microsoft 365 tools: Word, Excel, PowerPoint & Outlook.

EDUCATION AND EXPERIENCE:

Preferably a Bachelor's degree in Social Work, Gerontology, Human Services, Psychology, or Sociology.
Candidates with relevant work experience will also be considered.
Experience in engaging with a diverse senior and disabled population is beneficial.
