Refugee Support Services & Cash Assistance Case Manager - English/Spanish or English/Hatian Creole - Interfaith-RISE - Reformed Church of Highland Park Affordable Housing Corporation


Position Description
The Case Manager provides comprehensive and culturally appropriate case management services to longer-term resettlement clients.

The Case Manager will be responsible for collaborating with Direct Services Coordinators to ensure clients enrolled in the RSS and RCA programs achieve their goals and attain self-sufficiency.

This position must be fluent in English and Spanish or English and Haitian Creole. The RSS/RCA Case Manager Reports to the Director of Interfaith-RISE.

This position is located in Vineland, NJ.


Responsibilities Include

  • Provide comprehensive case management services to clients with barriers to selfsufficiency
  • Upon receipt of referral from internal and external sources, schedule an intake appointment, and arrange interpretation and transportation for the client, as necessary
  • Conduct intake, determine eligibility, complete assessment, and orientation, and assist client in developing their selfsufficiency plan
  • Refer clients to appropriate internal and external services and programs based on their needs
  • Provide linkages to agency, community and government services and monitoring referral completion including state benefits such as SNAP, Medicaid, WIC, SSI, and other available services within Tucson
  • Follow up with clients monthly to track their progress and update their selfsufficiency plan
  • Advocate for clients to ensure they receive culturally and linguistically appropriate services
  • Complete documentation in case management database systems in a timely manner
  • Follow all RCHP-AHC/Interfaith-RISE policies.
  • Input client data into RCA database for the storage of client files, program enrollment forms, monthly checkins, etc.
  • Work directly with Case Managers to enroll clients in the program, collect needed client information and data, and request RCA checks from AHC Bookkeeper.
  • Scheduling monthly meetings with each of RCA clients to check and ensure program compliance
  • Obtain copies of two pay stubs once a client achieves employment
  • Report any client issues to the Case Manager for appropriate followup
  • Case note client interactions in the agency's Apricot, case noting system

Qualifications

  • Demonstrated knowledge of working effectively in a team and independently;
  • Strong communication and organization skills
  • Knowledge of regional social services strongly preferred.
  • Lived experience as refugee or immigrant. In lieu of lived experience, experience working with refugees and/or immigrants; crosscultural or overseas experience accepted.

Language Skills:

  • Fluency in written and spoken English and Spanish or English and Haitian Creole
  • Crosscultural understanding and strong communication skills, both written and verbal

Certificates or Licenses:

  • Must have the ability to travel regularly throughout the service delivery area for home visits.

COMPENSATION:
This position is for 40 hours/week at a salary of $47,500.
