Description

Ensures the provision of active treatment as demonstrated by providing supervision and assistance to assigned caseload of individuals; implements training programs and provides services as specified in Individual Habilitation Plans (IHPs) and/or plans of care; documents the provision of training and services; provides transportation to work, recreational activities, medical appointments, etc.


ESSENTIAL FUNCTIONS:


  • Provides adequate supervision to assigned individuals to ensure safety as specified in IHPs and/or plans of care and company policy and procedures.
  • Assists individuals with daily living activities. Provides individuals with assistance as necessary in the areas of personal hygiene and grooming, money management, meal preparation, socialization, domestic (housekeeping) activities, etc.
  • Implements training and behavioral programs and relevant service objectives as outlined in IHPs and/or plans of care and maintains a thorough knowledge of each client's needs.
  • Provides transportation as needed to individuals for medical appointments, job attendance, recreational activities, etc.
  • Assists individuals in meeting dietary needs by implementation of dietary interventions as specified by physician and registered dietician. Assists individuals with meal preparation, as outlined in facility menus, to include proper food storage, proper food preparation and sanitary practices.
  • Supervises individuals in selfadministration of medication as specified per individual. Notifies appropriate personnel when medication needs to be reordered and implements documentation procedures as specified in policy.
  • Provides accurate and timely documentation substantiating the delivery of services to individuals as outlined in IHPs and/or plans of care and Reach Unlimited, Inc. policy.
  • Reports facility maintenance issues so that ICF-ID and/or HCS standards are met and a comfortable and safe living environment is provided.
  • Performs other duties as assigned.

  • Promotes and monitors cost containment measures by preventing unnecessary spending and thoughtfully reducing expenses in all areas of the program without negatively influencing the quality of client care.

MINIMUM QUALIFICATIONS:


  • Must have a high school diploma or equivalent.
  • Must have a current and valid Texas Driver's License.
  • Six months experience working with individuals with intellectual and developmental disabilities or related field is preferred.

PHYSICAL AND MENTAL REQUIREMENTS

  • Requires the ability to drive and safely operate a motor vehicle.
  • Requires the ability to grasp, move or carry items that may weigh up to 45 lbs without assistance.
  • Requires the ability to bend, stoop, kneel, reach, twist, and lift objects or position people.
  • Requires the ability to stay awake and alert while on duty.
  • Requires the ability to ascend and descend stairs in a multilevel residence.
  • Requires the ability to read and write in order to understand and communicate information and ideas.
  • Requires basic math abilities to calculate quantities and dosages of medication and to implement training objectives.
  • Requires the ability to stay focused on a critical task while experiencing distractions or interruptions.

ENVIRONMENTAL CONDITIONS

  • When loading and unloading a vehicle, an employee may be exposed to weather and unprotected from the sun, rain, wind, etc.
  • When performing housekeeping duties, an employee may have to handle certain cleaning products and chemicals, which when improperly mixed together, can cause an adverse reaction.
  • Clients served by the agency may have behavioral challenges which necessitate physical interaction by staff. By accepting the position, staff recognize the possibility of physical interaction with clients.
