Join our team and make a positive impact on the lives of others We are looking for a Community Outreach Specialist_ _for our team in Arizona.


  • This is a hybrid role with a required commute of 23 days per week to our office in Phoenix, AZ._

Department & Position Overview:


The Community Outreach Representative will serve as the key point of contact for all professional patient referrals within their territory.

Accounts will consist of hospitals, skilled nursing facilities, rehabilitation centers, senior centers, senior housing facilities and other identified opportunities.


This position will be responsible for growing the number of professionals within the territory and achieving the number of direct patient referrals to meet or exceed monthly referrals numbers.


Every Day You Will:


  • Develop and maintain strategic business partnerships in designated territory that result in driving consistent patient referrals
  • Drive 30 or more appropriate direct referrals each month, producing an outcome of 13 or more oncare consumers per month from your direct referral pipeline
  • Nurture relationships to be the "go to" thought leader and resource for consumer directed services locally
  • Familiarize yourself with the healthcare professionals ecosystem to determine how FreedomCare can best fit into serving their patients and caregivers
  • Leverage cold calling, appointments, in person meetings and marketing techniques to achieve goals
  • Lead informational presentations with hospitals, skilled nursing facilities, rehabilitation centers, senior centers, senior housing facilities and other identified opportunities
  • Attend and represent FreedomCare at local health and wellness events/expos
  • Will organize the FreedomCare team to participate in events that deepen our relationships such as volunteer activities.
  • Willingness to comply with company policies and government regulations
  • Ability to remain adaptable and resilient to all situations with an optimistic outlook and cast a positive shadow that is aligned with our culture and Core Values.
  • 5+ years of experience within business development or outbound sales
  • Healthcare field business development experience
  • Experience leveraging Salesforce or other CRM platform
  • Understand and appreciate the needs of healthcare professionals and the consumers they serve in order to provide appropriate assistance needed for consumer directed services
  • Appreciation for the hustle to execute a sale and the compassion about providing a quality customer experience
  • Demonstrated success in a metric/quotadriven environment with a focus on driving results
  • Allstar relationship builder with excellent communication and presentation skill
  • Strong analytical, organization and time management skills
  • Creative problemsolving skills to develop new ideas and outreach tactics
  • Proven ability to develop opportunities from ideation to execution
  • Ability to travel in the local territory up to 70% of the time as needed by scheduled appointments or to meet the customer needs and reliable transportation
  • Ability to travel outside of local region, for regional or national meetings
  • Access to reliable and consistent transportation
