- Assist AHH team in building and delivering dementia focused program to qualified seniors in their homes
- Follow AHH model to complete chronic care management and cognitive assessments to seniors in their homes
- Utilize telehealth and various methods to complete patient visits and calls
- Have passion and dedication in working with caregivers and family members living with those diagnosed with dementia
- Must have strong understanding of and ability to work with latest technology, EMR and various computer programs.
- Potential to deliver medical care in the patient's home. This involves driving or riding in your vehicle to the place of service.
- Ability to learn/perform comprehensive health assessments and diagnose and treat complex illnesses.
- Follow At Home Harmony's Coordinated Model of Care of Medical and Pharmacy.
- Use critical thinking skills and follow evidence-based standards of practice.
- Accurately and thoroughly document your patient encounter and ensure accuracy.
- Analyze test data to diagnose, treat and arrange appropriate follow up for the patient to ensure continuity of care.
- Educate patients and families on how to manage their health and well-being.
- Effectively navigate difficult conversations related to end-of-life issues and goals of care.
- Communicate effectively with patients, family, the medical power of attorney, other medical providers and all individuals involved in the patients care.
- Adhere to clinical and safety standards, protocols, and performance metrics.
- Provide care with compassion, empathy, and cultural competency.
- Maintain positive relationships with Care Concierge and At Home Harmony staff.
- Attend training sessions and clinical team meetings, as requested
- Serve as ambassadors for At Home Harmony and maintain professional demeanor.
- Minimum of two years of experience as an APP PA-C/NP in Geriatrics, Family Practice, Internal Medicine with a passion for working with the Geriatric population and added plus for experience in dementia care. *High-performing, self-starter new grads will be considered.
- Experience in home-based care a plus but desire to learn is welcomed
- Graduate with an advanced degree from an accredited institution
- Current unrestricted VA state license as a Nurse Practitioner or Physician Assistant
- Nationally board certified, current
- Current BLS required
- Prescriptive authority and DEA
- Valid drivers license with clean driving record
- Ability to lift and carry equipment up to 50 pounds
- Ability to walk up and down several flights of stairs easily

Our team of dedicated medical providers is growing, and were seeking experienced, high-performing and forward-thinking NPs/PAs to join our expansion team.
This position is a brand-new role for our company. As a recently accepted Medicare GUIDE program provider, we are excited to form a dedicated dementia focused medical team (NP or PA-C paired with a dementia care navigator). This role will predominantly be remote with phone/telehealth capability with limited home-based care/visits. Over time this may change as the dementia care patient panel is built and this could lead into a combination primary care/dementia focused role with home-based visits and telehealth needs.
We are looking for a medical provider who is interested in helping us build a geriatric dementia-based program under the guidance of CMS GUIDE model - which involves adaptability, flexibility, team-based work interest and proactive collaborative work ethic. This role has the potential to grow into a lead role that helps train similar providers as we continue to grow. Additionally, this role will work closely with other departments in our practice to administer other Medicare funded programs that align with outstanding dementia care and chronic care management. Youll collaborate with our interdisciplinary team of pharmacists, nurses, MAs, phlebotomists, clinical liaisons, utilizing the latest tools and technology to enhance patient outcomes and streamline provider workflows.
Our current team of Nurse Practitioners and Physician Assistants bring a wealth of knowledge in primary care/internal medicine/palliative/hospice/geriatric experience and have all contributed greatly to our continued success. They each have an interest in home-based primary care with the goal of caring for older adults with complex chronic conditions in their homes, group homes, and independent living.
