- Full-time position requiring 40 hours per week
- Flexibility in schedule to accommodate client needs
- Potential requirement to work at various CCAP locations
- Supporting treatment service plans with clients
- Meeting clients both at CCAP sites and in the community
- Coordinating services and referrals for clients
- Acting as a liaison between families, the legal system, and community resources
- Engaging at-risk individuals and coordinating crisis responses
- Assisting in the orientation of new clients
- Supporting clients in reducing high-risk behaviors and health factors
- Collaborating with colleagues to facilitate positive lifestyle changes
- Providing education on the behavioral health system
- Participating in public relations activities and professional development
- Attending weekly supervision meetings
- Other assigned duties
- Bachelor's degree in social work, human services, or related field
- Strong oral and written communication skills
- Experience with Electronic Health Records (EHR) is a plus
- Knowledge of health and human services landscape
- Ability to build relationships and work independently
- Effective communication with diverse stakeholders
- Proficient in various computer applications
- Ability to travel with a personal vehicle (driver's license and automobile insurance required)
- Proficiency in reading, writing, and public speaking
- Respectful interaction with diverse populations

JOB SUMMARY:
The CCBHC Behavioral Health Case Manager is a crucial member of the behavioral health/family health services team. This team is made up of various professionals like Behavioral Health Clinicians, Primary Care Providers, Nurses, and Social Workers. The primary role of the CCBHC Behavioral Health Case Manager is to work closely with Peers Support Specialists in the community to assist behavioral health clients and patients in accessing quality care. This involves outreach, building professional relationships, and coordinating care plans.
WORK SCHEDULE DEMANDS:
CONFIDENTIALITY OF INFORMATION:
The role involves access to program case management records and requires maintaining confidentiality as per State and Federal laws and CCAP policies.

PHYSICAL EFFORT / ENVIRONMENT:
The role involves prolonged sitting, eye-hand coordination, and some physical activity at community events. Travel between sites and interaction in group settings are also required.
