Summary

Prepares patients for examinations including basic diagnostic interviewing, rooming, administering medications and vaccines, wound care for wounds or incisions, and assisting with procedures within the physician office setting.

Provides care to designated patients and care coordination across the continuum. Provides education on prescription medications and clinical concerns under the direction of a provider.


Job Duties

  • Facilitates coordination of care across settings for a designated population ensuring that labs, studies, and referrals are completed and results discussed with the patient.
  • Performs patient care and procedures utilizing sound clinical judgment and approved policies/procedures/protocols/techniques, equipment, and supplies.
  • Cleans and sterilizes instruments and equipment and maintains supplies.
  • Assesses patient readiness to learn and ensures education materials are current and appropriate; evaluates effectiveness of patient education.
  • Intervenes to prevent, minimize, or correct actual or potential patient risks based upon observation of physiological, behavioral, and environmental factors.
  • Complies with monitoring and documentation of daily activities and expenditures as per standards set forth by accrediting bodies.
  • Acts as a resource for patient and family members, listens effectively, and gives and receives constructive feedback.
  • Participates in clinical meetings and other multidisciplinary quality service improvement initiatives.
  • Triages calls to appropriate level of care and provides healthcare advice/education using assessment skills, approved guidelines and protocols, and knowledge of selfcare practices.

Minimum Qualifications

  • Graduate of accredited Nursing program.
  • Ability to initiate, accept, and adapt to change within a healthcare setting.
  • Excellent time management skills.
  • Excellent computer skills including EMR, word processing, and Excel.
  • Knowledge of registration, insurance, managed care plans, medical authorization requirements, and referral coordination.
  • American Heart Association Basic Life Support

Preferred Qualifications

  • Bachelor's Degree Nursing
  • 1 year experience as a registered nurse.

Physical Demands
Lift and carry 25 lbs.
