    The Pediatric Inpatient Liaison will collaborate with the innovation center staff to evaluate the patient's medical or physical condition and to assess client needs at admission and as needed.

    Meets regularly with patients and families to support them throughout the inpatient stay.

    The Pediatric Inpatient Liaison will act as a liaison with families prior to admission to ensure a smooth transition to acute rehabilitation and what to expect in the next level of care.

    Consult with pediatric innovation staff and support clinicians to better serve our patients.
    Promotes therapeutic environment to innovation center staff with patients, families, and other caregivers.
    Assists in helping the clinical team build trust with patients and families. Provide outreach to patients and families before they arrive to better prepare them for the transition of care.
    Documents as appropriate and as needed in medical record. Assesses for and supports any special needs related to the culture of the patient/family/caregivers.
    Serve as a key resource to management, staff, and providers to improve patient satisfaction.
    Collaborate with leadership to identify and mitigate barriers as identified through patient complaints.
    Analyze customer satisfaction metrics and other performance analytics to influence accountability.
    Communicates patient findings to physicians, nurses, allied health and extended interdisciplinary team.
    Contribute to discharge planning as needed and assist with transition of care. May at times visit patients in outpatient and Day Rehab after discharge to ensure needs are being met.
    Be a point of contact for Get Well Network.
    Minimum 1 year of experience as a staff nurse. Knowledge of community resources, health care system and interdisciplinary functioning. Able to apply company policies and procedures to resolve routine issues. Able to follow practices and procedures in analyzing situations or data.

    Interpersonal communication, advocacy and analytic skills required to work with patients and families, and various medical professionals within and outside of the organization.

    Normal office environment with little or no exposure to dust or extreme temperature.
