- Provide child skill building as appropriate with an emphasis on community skills
- Transport child to visits as needed and appropriate
- Monitor length of stay of each child and progress toward discharge, and coordinate with the treatment team to remove barriers to treatment progress and discharge
- Assess need for utilize Flex Funds as appropriate to facilitate discharge to child's home community and provide for successful transition
- Begins discharge planning upon admission and continually assesses the discharge plan and discharge criteria for each resident
- Makes a referral to the discharge resource within 30 days of the recommendation being made by the treatment team
- Set up appropriate discharge service upon discharge, and in partnership with family will develop a Continuing Care Plan and a Crisis Plan and follow-up with client, depending upon the need of the family
- Implements lower level techniques of therapeutic crisis intervention (TCI) effectively, appropriately and safely
- Writes event reports correctly and in a timely manner, including antecedents, the event and the outcome
- Participates in TCI training and updates
- Provide after care services including home visits to provide in home parent skills training as needed and appropriate, coordination and linkage to community based services and supports, continually assess unmet needs and provide Flex Funds as needed and appropriate
- Master's Degree in human services field and two years experience in providing direct services to or in linkage for children with serious emotional disturbance to service, or a Bachelor's degree in human services and four years experience
- Ability to provide professional leadership and work well with an interdisciplinary team of professionals, paraprofessionals, and non-professionals
- Ability to relate to different social and ethnic groups, effective in developing therapeutic relationships with mentally ill children and their families
- Experience in managing hostile and aggressive behavior of children
- Effective communication skills verbally, in writing and by telephone
- Must have mobility to access multi-buildings, make home visits, and attend community meetings and visit other parts of the agency

Description
The Transition Coordinator provides transition coordination to children and families by obtaining a working knowledge of local resources, referral procedure and contracts, and providing continuous linkage to community based services and supports at discharge and during the period of transition when child's living situation is re-established in the community. The Transition Coordinator will augment the work of the treatment team in their function of development and implementing a discharge plan. The Transition Coordinator will monitor length of stay of each child and progress toward discharge. This may include the direct provision of in home parenting skills training, assessment of unmet needs and provision of Flex Funds, and transport of children on home visit as appropriate.
