Description

• Provide outreach to children and families utilizing the tenets of Wraparound, as supervised by Care Management Supervisor
• Assist the CMO team with duties, both internally and externally, as identified by the Care Manager Supervisor or Director of Care Management that will support the Child Family Team's plan
• Maintain positive working relationship with team members
• Demonstrate the knowledge and skills necessary to provide positive interactions and engagement that are appropriate to any agerelated needs of the children/families served
• Demonstrate respect and patience in engagement practices with families and youth
• Adhere to Camden County Partnership for Children Code of Ethics and Confidentiality Practices and maintain confidentiality with all documentation
• Complete Orientation and Training Process
• Observe all work place safety laws, regulations, standards and practices
• Maintain uptodateOutlook Calendar
• Other duties as determined by members of management

• Excellent leadership, communication, organizational and interpersonal skills
• High School Diploma
• Willing to accept feedback for best practices of engagement and alignment with youth and families served
• Must be energetic, creative, culturally competent, and responsive
• Computer literate with excellent writing skills
• Valid Driver's License
• Must have daily access to personal vehicle
• Must provide proof of registration and insurance for personal vehicle
