Job Summary:

We provide general adult and adolescent outpatient counseling, individual therapy as well as a professional outpatient group. Additionally, this location offers medication-assisted treatment, including Sublocade and MAT for pregnant women.

The Client Navigator is responsible for identifying and registering appropriate new clients and completing outbound follow up engagement calls in a timely manner.


Responsibilities:


  • Provides quality and efficient customer service for consumers seeking treatment for substance use within Pyramid Healthcare Inc.
  • Responsible for working with inpatient and outpatient teams to ensure coordination of care between levels of care and ensures a safe handoff through the referral process.
  • Acts as on the ground resource liaison as a client moves through the continuum.
  • Responsible for identifying and registering appropriate new clients and completing outbound follow up engagement calls in a timely manner.
  • Provides a high standard of customer service while working with qualified external providers.
  • Maintain census & processes referrals for HWH locations of Pyramid Healthcare Inc. (Pine Ridge Manor, Traditions and Gratitude Houses) while ensuring internal clients have priority.
  • Coordinates and collaborates with multidisciplinary teams for improved client care to ensure quality and comprehensive services.
  • Efficiently and effectively move clients through the continuum of care that best meets their needs.
  • Understanding and attention to Key Performance Indicators and impact on the Pyramid Healthcare system.
  • Other duties as assigned.

Technical Competencies:


  • Knowledge of substance abuse and mental health diagnosis and treatment continuum of care.
  • Working knowledge of the Microsoft Office suite, including Word, Excel, Power Point, Teams, Outlook as well as SmartSheets.
  • Excellent communication skills & effective interpersonal relationships

Education, Experience:

Bachelor Degree in a related field and at least 1 years of experience in substance abuse and mental health field.
