Abitare,Inc is case management company dedicated to behavioral healthcare and the Enhanced Case Management program. We are currently hiring a Housing Navigator to work full-time with clients experiencing homelessness or at-risk for homelessness in San Diego County


Requirements:

• A minimum of one year of experience providing case management support to people experiencing homelessness, or at risk of homelessness, including working with people experiencing chronic health issues, mental health needs, or with histories of substance use.
• A bachelor's degree in Social Work, Psychology, or Sociology is preferred.
• Possess a valid California driver's license and have access to properly registered vehicle.

Essential Responsibilities

• Manage a caseload of 3550 households
• Conduct intake and assessment activities with homeless individuals who are staying in motels as part of the Encampment Resolution Funding Motel Voucher program.
• Maintain ongoing facetoface visits with participants and tailor the intensity of provided services according need.
• Support participants being vouchered in motels including management of motel vouchers and assisting participants in navigating interactions with motel vendors to preserve their vouchers.
• Assist participants with linkage to interim and permanent housing resources, as well as any other community resources needed to promote wellness and stability including medical, mental health, and/or substance use services, employment, etc.
• Provide traumainformed services that align with Housing First and Harm Reduction philosophies.
• Transport participants as needed to essential appointments and conducts visits within the community in San Diego County.

Pay:
$46, $58,000.00 per year
