Responsibilities
*Writes the nutrition service plan for Diabetic patient, hyperlipidemia and Weigh loss patients in consultation *Develops or identifies a nutrition screening instrument to assess patients. Trains staff in the administration and interpretation of the screening instrument.

  • Follows up on screenings indicating nutritional risk and provide consultation and training to patients. Assures families with nutrition related needs are referred to community resources that provide ongoing nutrition services. *Ensures that dietary modifications are accommodated for patients with a nutritional concern based on a medical diagnosis and/or a disability, or religious/cultural preference.

Available shifts and compensation:
We have available shifts all days of the week. Compensation depends on your experience.
