BronxWorks is hiring Eight (8) Case Manager for the Access to Benefits Program.

RESPONSIBILITIES

Provide one on one case management services to building/neighborhood residents.
If applicable conduct site visits to rapid rehousing DV clients
Participate in all mandatory agency trainings.
Attend internal and external agency/contract meetings.
Participate in monthly staff meetings or more frequently as required.
Meet monthly with Program Director/Assistant Program Director for exchange of ideas and written feedback.
Submit monthly reports to supervisor.
Perform additional duties as assigned

Program Specific Responsibilities·

The case manager will carry a case load
S/He will direct the client to the appropriate service or program and will complete a prescreening/intake
The case manager will enter information into Salesforce databases confirming whether the referral was accepted
The case manager will enter updated information into Salesforce database concerning missed appointments, declined services, etc
The case manager will continue to enter information regarding the status of the clients' case
The case manager will attend meetings and conduct presentations
The case manager will assist the Program Director writing reports to funders
The case manager will conduct ongoing monthly or bi-monthly home visits with staffAll other duties as requested by the supervisor

QUALIFICATIONS

Bachelor's degree.
Proficiency in English required; proficiency in a second language preferred.
Proficiency in modern business communications including personal computers, electronic mail, voicemail, facsimile and copier equipment.

PHYSICAL REQUIREMENTS

Ability to use a computer for prolonged periods
Ability to occasionally lift and/or move up to 10 pounds
Ability to stand, walk, or sit for long periods of time
Ability to climb five flights of stairs, if required to conduct home visits and/or fieldwork
Ability to bend and retrieve objects and/or documents
Ability to travel in the boroughs of New York City and its adjacent counties via public transportation
Ability to travel for a minimum of 1 mile within a reasonable amount of time and in all types of weather, including inclement weather, if required to conduct home visits and/or field work
