CORE JOB RESPONSIBILITIES:

The position will be primarily focused on areas that impact medical, behavioral health and substance use disorders (SUD).

This position is also responsible for assessing patient social determinants of health (SDOH) and evaluating the impact of services on health outcomes, assisting with outreach activities in the community.

Core Responsibilities:

• Conduct outreach and engage with community members to assess their health needs
• Provide education and resources on various health topics, including addiction counseling and Medicare benefits
• Assist individuals in navigating the healthcare system and accessing appropriate services
• Conduct data collection and maintain accurate records of interactions and outcomes
• Collaborate with healthcare professionals to develop and implement individualized care plans
• Advocate for the needs of community members and connect them to relevant support services
• Participate in public speaking engagements to raise awareness about health issues

Qualifications:

• CHW Certification preferred
• Must be bilingual, Spanish & English (spoken and written)

Skills:

• Strong communication skills, both verbal and written
• Knowledge of medical terminology and healthcare systems
• Ability to build rapport and establish trust with diverse populations
• Proficiency in motivational interviewing techniques to promote behavior change
• Excellent organizational skills for data collection and record keeping
• Ability to work independently and as part of a multidisciplinary team
• Cultural sensitivity and understanding of social determinants of health

This role requires a passion for community health, strong interpersonal skills, and the ability to work effectively with individuals from diverse backgrounds.

The Community Health Worker will play a vital role in improving the overall health outcomes of the community by providing education, support, and advocacy.

Pay:
$ $24.00 per hour

Expected hours: 40 per week
