About us


Queens Centers for Progress, an Agency providing person centered services for adults and children with developmental and intellectual disabilities is seeking a compassionate and dedicated licensed Social Worker for our Article 16 Clinic.


SUMMARY:


Under the supervision of the Article 16 Clinic Director, deliver planned intervention to persons with intellectual Developmental Disabilities (IDD) to resolve or ameliorate social problems encountered.

The method of service delivery is through individual, and/or family counseling.


DUTIES:


  • Conduct intake/screening to potential clients and assist in defining the presenting problem or treatment
  • Conduct psychosocial evaluations and arrange for additional clinical evaluations needed by the client
  • Provide ongoing individual and /or group counseling to at least 30 assigned cases
  • Develop Individual's annual treatment plan based on a thorough needs assessment and review TX plan every 6 months
  • Update and maintain case records in accordance with Part 679 regulations
  • Provide information/referral services to persons with developmental disabilities and their families
  • Provide family counseling services to those individuals duly admitted to the clinic experiencing familial conflict and stressors.

QUAIFICATIONS:

To be successful in this role, you should have the following qualifications:

  • Master's Degree in social work
  • One year work experience in OPWDD
  • Valid NYS Social Work License
  • Excellent communication and interpersonal skills

Pay:
$ $34.50 per hour

Expected hours: 35 per week
