Responsibilities:

Wound Assessment and Planning: Conduct assessments of patients' wounds, considering multiple factors such as type, size, location, and underlying health conditions.
Collaborate with the interdisciplinary team to develop individualized wound care plans, setting appropriate goals and interventions.
Continuously monitor and document the progress of wounds, modifying treatment plans as necessary.
Perform specialized wound care procedures, such as cleansing, debridement and dressing selection
Administer topical medications and wound adjunctive therapies as prescribed by physicians.
Educate patients, families, and caregivers on proper wound care techniques and prevention strategies.
Ensure compliance with infection control practices and maintain a clean and sterile wound care environment.
Be a resource person for staff members, providing guidance and education on wound care best practices.
Maintain accurate and detailed documentation of wound assessments, interventions, and outcomes.
Communicate effectively with patients and their families, addressing their concerns and providing emotional support.

Qualifications:

Valid registered nursing (RN) license in NY State
Minimum of 1 year of experience as a wound care nurse, preferably in a skilled rehab nursing home or LTC setting
Proficient in wound documentation and understanding of electronic medical record systems.
Strong problemsolving, and decisionmaking skills.
Excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare professionals.
Ability to work collaboratively within an interdisciplinary team.
Strong attention to detail and ability to multitask in a fastpaced environment.
Commitment to providing compassionate and patientcentered care.
