Responsibilities:


  • Wound Assessment and Planning: Conduct assessments of patients' wounds, considering multiple factors such as type, size, location, and underlying health conditions.
  • Collaborate with the interdisciplinary team to develop individualized wound care plans, setting appropriate goals and interventions.
  • Continuously monitor and document the progress of wounds, modifying treatment plans as necessary.
  • Perform specialized wound care procedures, such as cleansing, debridement and dressing selection
  • Administer topical medications and wound adjunctive therapies as prescribed by physicians.
  • Educate patients, families, and caregivers on proper wound care techniques and prevention strategies.
  • Ensure compliance with infection control practices and maintain a clean and sterile wound care environment.
  • Be a resource person for staff members, providing guidance and education on wound care best practices.
  • Maintain accurate and detailed documentation of wound assessments, interventions, and outcomes.
  • Communicate effectively with patients and their families, addressing their concerns and providing emotional support.

Qualifications:

  • Valid registered nursing (RN) license in NY State
  • Minimum of 1 year of experience as a wound care nurse, preferably in a skilled rehab nursing home or LTC setting
  • Proficient in wound documentation and understanding of electronic medical record systems.
  • Strong problemsolving, and decisionmaking skills.
  • Excellent interpersonal and communication skills to effectively interact with patients, families, and healthcare professionals.
  • Ability to work collaboratively within an interdisciplinary team.
  • Strong attention to detail and ability to multitask in a fastpaced environment.
  • Commitment to providing compassionate and patientcentered care.
