Our Dutchess Division is currently seeking full time applicants for our Dutchess Supported Housing Program located in our Poughkeepsie office.

This is a direct care/entry level position.


Job Title:
Housing Specialist


Hours of Work:
Full-time, 40 hours per week. Non-Exempt.

Minimum Qualification:
High School Diploma or equivalent, and one year experience in Human Services.


Functions:

  • 1. Assist residents in finding and keeping preferred housing.
  • 2. Establish and maintain good rapport and working relationships with residents.
  • 3. Contribute to the formulation and implementation of individual housing support plans.
  • 4. Aid and encourage residents to meet the established goals of their housing plans.
  • 5. Encourage resident decisionmaking.
  • 6. Report problems and behavioral changes to supervisor.
  • 7. Provide information, referral and advocacy.
  • 8. Work collaboratively with all internal and external entities, and participant collaterals includes family, significant others, and medical & behavioral health treatment providers
  • 9. Ensure the protection of all client civil liberties, rights, and property.
  • 10. Attend all staff meetings.
  • 11. Prepare/maintain documentation as per regulatory requirements for electronic client case records (This may include, but not limited to, Service Plans, Service Plan Linked Notes, Functional Assessments, Health Related Information, etc )
  • 12. Responsible for the accurate and timely completion of case record documentation in the Agency electronic health record.
  • 13. Participate in supervisory meetings.
  • 14. Visit apartment sites at least once per month.
  • 15. Oncall responsibilities, where applicable.
  • 16. Ensure clients are transported to needed services in a timely manner, if applicable.
  • 17. Complies with HIPPA requirements to ensure confidentiality and the protection of all client civil liberties, rights, and property
  • 18. Comply with all workplace policies & procedures.
Other Duties: 1. Attend/participate in any/all required trainings related to the responsibilities of the position. 2. Performs duties assigned by the immediate supervisor or other management staff, as required.


Other requirements:

  • Successful clearance by the Justice Center and New York State Office of Children and Family Services Statewide Central Register Database Check is also required.
