As part of our dedication to excellence, we are seeking a dynamic and organized Admissions Coordinator to manage referrals and admissions through the Allscripts and NAVI Health portals.
Position Overview:
The Admissions Coordinator plays a critical role in facilitating the admission process for prospective residents and their families.
This position requires strong communication skills, attention to detail, and the ability to work collaboratively with internal and external stakeholders to coordinate admissions effectively.
Key Responsibilities:
- Manage referrals and inquiries received through the Allscripts and NAVI Health portals.
- Review referral information and gather necessary documentation for admission consideration.
- Coordinate with healthcare professionals, discharge planners, and referral sources to obtain relevant medical records and documentation.
- Conduct preadmission assessments and screenings to evaluate prospective residents' needs and eligibility.
- Communicate effectively with prospective residents, families, and healthcare providers to provide information about our services and answer any questions.
- Collaborate with the interdisciplinary team to ensure smooth transitions and continuity of care for new admissions.
- Maintain accurate and uptodate records of admissions, discharges, and bed availability.
- Follow established admission policies, procedures, and regulatory requirements.
- Provide support and assistance to residents and families throughout the admission process.
- Participate in meetings, trainings, and quality improvement initiatives as required.
Qualifications:
- Previous experience in admissions coordination, healthcare administration, or related role preferred.
- Familiarity with electronic health records systems, including Allscripts and NAVI Health, preferred.
- Strong organizational and time management skills with the ability to prioritize tasks effectively.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively in a fastpaced environment.
- Knowledge of healthcare regulations and compliance requirements.
- Compassionate and customerfocused approach to service delivery.
