MUST HAVE INTAKE COORDINATOR EXPERIENCE

The Responsibilities of the Intake Coordination:

Proficient in Axxess /Kinser well sky EMR software.
Schedule and staff all patient visits within prescribed discipline and visit frequency requirements.
Verify the primary care physician who will assume responsibility for home health oversight.
Ensures authorization for service is confirmed for all admissions, as required by payers.
Ensures appropriate physician orders for referrals are obtained, as indicated.
Work with community referral sources, families, and Agency staffs to problem solve and identity services available to the client either through Agency services or the community.
Assist with reviewing of patient medical records, visit notes, physician orders for compliance with federal, state and local laws, Accrediting Organization standards and Agency policies and guidelines.

The Qualifications:

Excellent customer service skills
0 2 years of home health care experience required
Must have a vehicle, driver's license, and appropriate automobile insurance coverage.
Communication, interpersonal, and organizational skills necessary to complete job Responsibilities
Experienced with Axxess AgencyCore (EMR Software) Experience

Home Healthcare: 2 or more years (preferred)

This is a full time position that includes a work schedule Monday
