Position Purpose:

As a Certified Home Health Aide (CHHA), you will be providing personalized care to patients ensuring their well-being and assisting in their rehabilitation under the guidance of a Registered Nurse and as per the prescribed treatment plan. Your role involves creating a safe home environment and delivering age-specific care from infants to adults.


What You Will Do:

  • Provide comprehensive personal care adhering to the care plan while considering patient preferences.
  • Assist patients with personal care tasks, prioritizing their safety.
  • Support patient mobility and perform simple procedures recommended by the nurse.
  • Handle patient transfers using proper techniques and equipment for their comfort and safety.
  • Offer basic meal preparation, laundry, housekeeping, and other directed services.

Minimum Qualifications:

  • Current Certification as a Home Health Aide with Iowa Registry.
  • Availability for weekend and holiday calls and home visits.
  • Possession of a valid driver's license and auto insurance.
  • At least one year of health aide experience, preferably in Home Care/Hospice.
  • High school diploma or equivalent.
  • Completion of BLS certification within 3 months of employment.
  • Good communication, observational skills, and ability to work independently.
  • Comfortable with shared leadership and collaborative work environment.
  • Training in hospice care if applicable.
  • Demonstrated integrity, honesty, and compassion in line with Trinity Health's values.
