Once you complete your orientation, your schedule will typically involve rotating between three 12-hour shifts one week and four 10-hour shifts the following week, with every other weekend included
What you'll be doing:
We provide quality care regardless of circumstance to a variety of different socioeconomic and cultural backgrounds.
You'll team up with external medical professionals and referral sources, as well as our own clinical teams, to organize admissions into our inpatient hospice care center.
Building connections with referral sources to improve the admission process and offer community support.Coordinating with outside vendors, such as transport companies, to schedule patient arrivals smoothly.
Our Hospice Care Center in Wilmington has 18 beds.
You'll be responsible for keeping track of the number of patients in all 18 beds, working closely with local hospitals, and Lower Cape Fear LifeCare's inpatient palliative care team, outpatient nurses, social workers, and clinical liaisons to ensure our beds are utilized appropriately.
Admission visits take approximately one hour.During the admission, you'll complete necessary initial assessment documentation and develop a plan of care.
Document regulatory eligibility for general inpatient admission criteria for things such as:
Pain control.
Acute or chronic symptoms that need to be managed with a higher level of care.
Respite admissions.
You'll need to be comfortable educating the caregivers and family. This might include things on how to identify signs and symptoms, medication information, and what to expect at end-of-life.
Real-time documentation takes place during admission.
You will be part of an Interdisciplinary Team (IDT) consisting of a physician, nurse practitioner, registered nurse, medical social worker, chaplain, and a CNA who are all on site.
Working closely with and documenting orders from Nurse Practitioners and Physicians to plan patient admissions.Assign patients to available rooms and staff nurses.
Help staff with patient discharges for respite or short term stays back to their home.
Responsible for ordering controlled substances weekly from contracted pharmacies
JOB SPECIFICATIONS 1 Education:
Graduate of an approved school of nursing, either through an AD, Diploma, or BSN program
2 Licensure / Certifications:
Current license to practice professional nursing in the State of North Carolina / South Carolina
3 Experience:
Two years experience. Prefer hospice or home care. #LCFL1
