Description
Director of Continuous Care Services - full-time managerial position

The Director of Continuous Care Services will oversee a variety of programs in this multifaceted role which will include the following:

  • Palliative Care
  • Chronic Care Management
  • Medicare Wellness Visits
  • Transitional Care
  • Patient Navigators
  • The Access Center
This is a full-time exempt level management position.

This role will involve a great deal of program development and management as some of these programs are in their beginning stages of operations and development.

Previous experience with performance improvement activities and business development and knowledgeable of Joint Commission standards are preferred.


This position will supervise all staff within the areas noted above which may include oversight of nurses, patient access representatives, nursing assistants, medical providers and patient representatives.

They will work with many other areas of the organization in the coordination of care through these service lines.

This position may work in conjunction with both internal and external health care providers to develop robust service options within these designated areas.

This position will work to streamline processes for patients that require these services and the support that they will offer to them.
