    Description

    The Physical Therapist in Home Health has the crucial task of assessing and evaluating patient care needs related to functional status, mobility, endurance, pain, and other medical conditions. Through this evaluation, the Physical Therapist contributes towards determining a treatment plan, carrying out interventions focused on improving the patient's well-being, and monitoring the patient's progress.

    • Evaluates the patient's functional status, body systems, and physical therapy needs; collaborates with the physician to develop the therapy plan.
    • Delivers services within the defined scope of practice as per state laws, aligning with the plan of care and in coordination with other healthcare team members.
    • Monitors and communicates the patient's response to treatment and any changes in condition to the nurse supervisor and/or physician.
    • Provides treatment to alleviate pain, enhance function, and maintain maximum functionality using evidence-based techniques.
    • Educates the healthcare team, patient, and family/caregivers on the care plan, limitations, and progress towards goals.
    • Visits patients within the service area and neighboring LHC agencies as required.
    • Completes progress notes for each visit as per Plan of Treatment and LHC policy.
    • Conducts patient evaluations, develops the PT plan of care, and promptly reports evaluation outcomes and care frequency.
    • Participates in agency-recommended staff meetings and committees.
    • May oversee home health aides and provide supervision to the PTA following LHC policy.
    • Engages in weekend rotations to serve their agency and nearby LHC agencies as necessary.
    • Prepares discharge and summary reports on the patient's condition regularly.
    • Offers in-service education as requested by agency leadership.
    • Abides by LHC Therapy Standards of Practice to deliver high-quality patient care.
    • Pursues ongoing education to stay abreast of current care standards and maintain licensure requirements.
    • Performs any other assigned duties.

    Licensure Requirements

    • Current Physical Therapy licensure in the respective state
    • Valid CPR certification
    • Must possess a current driver's license, vehicle insurance, and have access to a reliable vehicle
