Description

The Personal Health Coordinator is responsible for overseeing health coordination services to members of the Seabury At Home program, enabling members to remain in their own homes by establishing individual wellness profiles to assure pro-active intervention when necessary through the means of temporary or long-term rehabilitation services.

2+ years related experience and Masters Degree in Social Work or LCSW preferred. This is a full-time position working 40 hours/wk with a rotating on-call schedule.


Duties:


  • Available to members on 24/7 basis via cell phone.
  • Participate in new member closings and onboarding
  • Coordinates with internal and external sources/contacts to assure that members have access to clinical, social, education, fitness, and any other services that members need.
  • Responsible for managing the SAH membership database and maintaining a member directory.
  • Participate in all aspects of planning for SAH social events, which often includes collaborating with marketing, culinary and other Seabury departments.
  • Ability to develop interpersonal relationships with members.
  • Handles family relationships in all stages of membership.
  • Maintains monthly, quarterly and annual member benefits utilization information.
  • Develops health coordination plans and oversees the implementation of plans.
  • Meets goals and objectives established by Director, Community Outreach Services.
  • Assists/Develops and implements health/service coordination policies and procedures.
  • Conducts Lifestyle Wellness Assessments for members.
  • Provides counseling and social service support to At Home members.
  • Assists with marketing/networking functions.
  • Supports the philosophy and mission of Seabury.
  • Conveys a courteous, professional and caring manner.
  • Performs other related duties as assigned.

Pay:
$60, $75,000.00 per year
