    Job DescriptionJob Details Our client is seeking an Outreach Specialist to engage with community partners and drive referrals for the PACE program in Los Angeles.

    The ideal candidate will have experience in healthcare sales, geriatric experience, and preferably bilingual. This individual educates the community partners, prospective participants, families, and or caregivers about the PACE program.

    The Outreach Specialist helps keep the frail elderly to live independently at home and continue to enjoy the quality of life.

    Full Time Permanent Position for An Outreach SpecialistHybrid
    • In field work 2 3 days/week, Onsite at Clinic Center 2 3 days/weekMonday
    • Friday, 8AM 5PMCompetitive Salary + Enrollment Incentive PlanResponsibilitiesDevelop and maintain relationships with healthcare professionals and community partnersCollaborate with Enrollment Specialist to ensure successful enrollmentOrganize and conduct events to promote the PACE programMaintain CRM software recordsAdhere to DHCS and CM regulationsDevelop marketing plans for enrollmentCollaborate with management on retention strategiesAttend community and healthcare events to increase censusIdentify opportunities to enhance referral processRequirementsBachelor's Degree or Higher2+ years experience in Community Relationships and GeriatricsHealthcare Sales experienceMust have reliable transportation, good driving record and auto insuranceBilingual in English and any of the following languages: Spanish, Korean, Cantonese, Mandarin, or Vietnamese.
