Position Summary:

We are seeking a skilled and compassionate Nutritionist to join our team.

As a Nutritionist, you will be responsible for providing expert guidance and support to individuals seeking to improve their overall health and well-being through proper nutrition.

Your role will involve conducting patient assessments, developing personalized nutrition plans, and offering health coaching to help clients achieve their goals.


Position Responsibilities:


  • Promotes healthy eating habits through the provision of nutritional assessment, diagnostic therapy, and counseling services, to educate people living with chronic diseases, specifically those who have diagnosed with diabetes or are at risk for diabetes to improve and prevent further complications of the disease.
  • Develops an evidencebased treatment program promoting healthier eating habits.
  • Educates patients and staff regarding necessary lifestyle modifications involved in diabetes management and control.
  • Counsels patients on an individual basis regarding nutrition and behavior changes for desired outcomes.
  • Conducts group sessions for weight management and diabetes selfmanagement education, including education on the planning and preparation of food at home for these patients.
  • Performs patient outreach to help meet HEDIS measures. Coordinates with supporting departments (i.e. referral) on nutrition services covered by insurance and performs patient outreach when applicable.

  • Develops individualized plan of care for patients and documents the plan of care in the medical record.
  • Participates in quality improvement activities related to disease management.
  • Participates in inservice trainings, selfcare management programs and other teaching/learning activities.
  • Other duties as assigned.

Position Requirements:


Education:


  • Master's degree in nutrition/dietetics required.

Experience:


  • Minimum two (2) years' experience as a registered dietitian nutritionist.

Knowledge and Skills:


  • Bilingual preferred
  • Computer Literacy required.
  • Good communication and writing skills.
  • Good organization skills and the ability to work independently as well as with a team.

Liccenses/Certifications:


  • Certified Dietitian/Nutritionist by the State of New York
  • Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM) credentials preferred.

Physical Demands:


  • Involves standing, walking, sitting, talking, hearing and bending
  • Involves transporting patients, material, or equipment, or lifting, moving, or carrying heavy (over 30 lbs.) materials for intermittent periods throughout the day.
  • Work may require evening, night, holiday, or weekend assignments on occasion
  • Occupational risks include cuts, burns, exposure to toxic chemicals, injuries from falls, or back injury sustained with assisting in moving/lifting/positioning patients, equipment or materials
  • OSHA Category II no highrisk procedures, no procedures to obtain or process body fluids / minimum anticipated exposure. Work does not involve exposure to blood, body fluids, or tissue, but may require performing unplanned exposure tasks
  • Position may require the use of safety equipment, such as HEPA mask, for infection prevention
