LilyCare Hospice is looking to hire caring and compassionate LNA's to care for our hospice patients.

NH state Licensed Nursing Assistant Licance required

The LNA/HHA works under the direct supervision of the RN Case Manager and/or Clinical Manager to:

  • Assist with personal care of patient, i.e., bathing, feeding, bed linen care, mouth care, vital signs, transferring and shaving.
  • Document in patient's chart.
  • Follow infection control and safety measures.
  • Assist with patient/family teaching concerning patient care and care for the caregiver.
  • Bereavement activities as requested and/or able.
  • Clean and monitor supplies.
  • Attend and participate in staff and patient review meetings when able.
  • Maintain and uphold the Hospice Code of Ethics.
  • Keep Patient Care Coordinator/Nurse Case Manager informed of condition of patient/family.
  • Maintain appropriate number of supplies at patient's residence.
  • Provide emotional support to patient/families.
