SUMMARY:

The Comprehensive Perinatal Services Program (CPSP) Coordinator will coordinate with a multi-disciplinary provider
team in the delivery of Comprehensive Perinatal Services in a community health setting.
The CPSP coordinator is responsible for navigating and supporting patients through their obstetric care and CPSP

service components:
psychosocial and nutrition health education, client orientation, initial assessment, individualized care
plan including intervention and reassessments, and postpartum assessment and care. The goal for the CPSP coordinator
is to improve perinatal access to care and pregnancy outcomes through enhanced prenatal care.


ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:


  • Supports and implements the organization's vision, mission, and values.
  • Performs all job functions in a professional, courteous and timely manner. This includes answering all phone calls,
  • Fosters and promotes a culture of service excellence and self-accountability.
  • Responsible for implementations and integration of CPSP Program including nutrition psychosocial and health
education assessments, interventions, and perinatal education with basic obstetrical care.

  • Responsible for ensuring the integrity and quality of the health services delivered in Comprehensive Prenatal
Services and in compliance with Federal, State, and L.A. County Standards and individual site policies, procedures,
and protocols.

  • Works cohesively with pediatric and dental departments to ensure patient and newborn care coordination.
  • Maintains all program logs and statistics for internal and external review and reporting.
  • Provides follow-up and completes proper documentation of post-partum delivery and lactation education.
  • Follows up on relevant laboratory tests.
  • Participates in Continuous Quality Improvement plans for site and program.
  • Establishes rapport with WIC and external agencies to aid in patient support.
  • Provides support to OBGYN provider(s) as needed.
  • Performs all other duties as assigned.
  • Keeps up with all necessary CPSP certifications.
  • Must be willing and able to work at all locations as needed to meet patient care needs.
  • Must be willing and able to work all business hours including evenings and weekends.

EDUCATION/EXPERIENCE:

High School graduate or GED equivalency. Completion of Medical Assistant program. CPSP certification. One year of
Obstetrics experience.

  • Bilingual in English and Spanish preferred

LANGUAGE SKILLS:

Ability to read and interpret documents, such as policies and procedures, benefits information, board minutes, routine
mail, simple contracts, and instruction manuals. Ability to compose routine reports and correspondence. Ability to speak
effectively with employees, visitors, and management.


REASONING ABILITY:

Ability to exercise common sense in carrying out instructions furnished in written, oral, or diagram form and in other
daily situations that arise. Ability to deal with problems involving several concrete variables in standardized situations.
Ability to make decisions and execute them in a timely manner in order to produce a positive outcome.


OTHER SKILLS AND ABILITIES:


  • Performs as a self-starter who needs no reminders of duties and tasks that need to be completed.
  • Ability to organize and prioritize work with minimum supervision.
