Reach-Up Care Manager
Provide integrated service planning, coordination and system navigationfor individuals enrolled in Reach-Up that are affected by mental healthand/or substance use disorders.
Responsibilities:
Navigate systems to increase ability to engage in affectivetreatment
Provide screening and ongoing assessment
Administer or refer to inter-agency resources for specializedscreening and assessments for substance use, mental health, andtrauma
Facilitate and monitor treatment plans as appropriate includingnecessary treatment team meetings
Assist to develop, place, monitor and evaluate participant\'sprogress in substance use and mental health services as it relatesto the Reach Up participant\'s wellness goals.
Requirements:
A combination of education and relevant experience working withindividuals coping with substance use and mental health issues
Knowledge of community resources
Valid driver\'s license and use of a personal vehicle
