Supervisor - Level - Community Health Services - Adult Care Management

Salary range - $24.15-$27.18/hr. FLSA (Non-Exempt)

QUALIFICATIONS:


  • Bachelors in Human Services field required
  • Three years' experience working in human service field
  • Two years supervisory experience required

Skills

  • Good problem solving skills
  • Ability to communicate effectively
  • Good leadership and team - building skills
  • Ability to assign and delegate tasks
  • Good organizational skills
  • Ability to work independently
  • Familiar with computer programs (e.g. Word, Excel)
  • The ability to climb stairs, bend, stoop, sit, stand, reach and lift items weighing 40 lbs.
  • Ability to legally operate a vehicle in NY State

MAJOR FUNCTIONS:


  • Understand and implement all agency and program policies and procedures
  • Maintain compliance with all applicable regulations
  • Assign staff and maintain adequate staff coverage to meet client/program needs
  • Monitor, evaluate and provide direction to staff
  • Assure that all staff training requirements are met
  • Convene periodic staff meetings, and submit minutes to manager
  • Monitor program expenditures and supplies
  • Assure that routine building upkeep and maintenance is done on a regular basis, if applicable
  • Maintain accurate accounts/program fees
  • May be assigned to participate in rotating afterhours/weekend on-call coverage
  • Provide input to Manager on staff training and performance issues
  • Establish and monitor relationships with persons receiving services and assure that individual needs are met
  • Provide oversight/referral/coordination of clinical services
  • Represent agency and program in the community and with other service providers
  • Assure adherence to emergency, safety and evacuation policies
  • Establish relationship with all residents, family members and advocates as appropriate
  • Monitor and advocate for individual rights as needed
  • Participate in safety training and development activities
  • If applicable, ensure that medications/treatments are provided as per doctor's orders
  • If applicable, provide supervision to Behavioral Health Professional staff.
  • Inform Manager of all pertinent resident and program issues on a regular basis
  • Participate in meetings as requested by Manager
  • Report and document incidents per regulations
  • Understand and follow all program procedures
  • Participate in and successfully complete all required staff training and development activities
  • Coordinate services with other community providers as needed
  • Regular and substantial contact with individuals receiving services
  • Perform other job related activities as directed by Manager.
