- Every Wed; 8a5p (clinic)
- Every 2nd Fri of the month 8a5p (Telemed Clinic)
- Every 4th Fri of the month 8a5p (clinic; 1st patient at 9a)
- Travel, lodging, and malpractice insurance covered
- Active AZ License or IMLC
- BC/BE
- AZ DEA
- ACLS
- At least 1 yr of experience
- COVID & Flu Vaccination proofs/exemptions
