JOB DESCRIPTION

Job Title:
Peer Support Specialist


Department:
Behavioral Health Services (ECI-OMH Project)


Reports to:
Project Manager


RESPONSIBILITIES:


  • Provide peer support services.
  • The PRNs will complete onsite training and will also attend Certified Peer Recovery Specialist training before beginning service delivery.
  • Provide outreach and recruitment, education and referrals, mentoring, and ongoing support and followup to ensure clients remain in programming and receive the services they need.
  • Conduct peer support groups to increase retention in services and peer support.
  • Conduct home visits and street outreach to project participants who have dropped out of the project or who have relapsed and invite them to return for follow up.
  • Create and maintain confidential records of contacts and referrals made and submit them weekly to the Senior Case Manager and Project Coordinator.
  • Create and maintain confidential outreach progress notes.
  • Document all services in accordance with the dictates of funding contracts.
  • Participate in weekly staff meetings
  • Prepare and submit reports to the Project Manager as required.

JOB QUALIFICATIONS:


  • Previous formal training in substance abuse and hepatitis prevention, care or treatment is preferred.
  • HIV/hepatitis education, public speaking and street outreach experience preferred.
  • Experience in peer counseling, experience educating those at high risk regarding substance abuse and HIV/hepatitis harm reduction.

Because we specialize in Infectious Diseases, you may be called upon by the agency to assist with COVID 19 prevention-related task.


Pay:
$ $27.50 per hour

Expected hours: 35 per week
