- Connecting with patients who are initiating remote patient monitoring programs.
- Serving as the primary point of contact for patients enrolled with PrimoCare, guided by their healthcare providers.
- Assisting patients in completing assessments using iPads.
- Regularly engaging with assigned patients to ensure they stay on track with their treatment plan.
- Scheduling consistent monthly check-ins with patients.
- Monitoring patient data to ensure their well-being.
- Empowering and educating patients by sharing valuable health information.
- Collaborating with the PrimoCare team through informative meetings to stay updated on the latest developments.
- Working closely with the sales team to ensure key performance indicators are effectively achieved.
- Assisting with patient interactions and data collection.
- Strong ambition, self-motivation, and self-discipline.
- Highly organized, efficient, detail-oriented, and adept at multitasking.
- Exceptional communication skills (both written and verbal).
- Proficiency with technology, including the ability to navigate computer platforms and familiarity with Excel.
