Client Services Coordinator

Hourly Rate:
$15 - $18/HR


Status:
Hourly, non-exempt


Empowering Minds is a human services agency dedicated to improving the social functioning of adults, adolescents, and children within a framework that recognizes the primary importance of family and community.

To this end, the agency's mission to provide comprehensive mental health care to families and children utilizing the services of qualified professionals implementing individualized rehabilitation plans that assist all persons in obtaining their desired goals.

To honor this commitment, we are involved in partnering with other agencies aimed at providing better health care.

These include consultation and service provisions to area schools, other community agencies, groups, and individuals, as well as other cooperative prevention efforts.


The duties and responsibilities of the Client Service Coordinator position includes but is not inclusive of or limited to the following:

  • Ensure agency compliance with COMAR regulations, contract terms, policies, and procedures.
  • Completing RSVP, monthly calendared and transportation logs for PRP Groups.
  • Conduct note reading
  • Process Discharges
  • Conduct outreach calls as needed
  • Process referrals
  • Manage rehab plan account
  • Assign Intakes and Annual Assessments
  • Request DLA20's
  • Participate in mandatory staff meetings as requested

Minimum Qualifications:
AA degree preferably in Human Services field One year of experience within the mental health/social services/human services field Strong organizational skills Excellent time management skills


Salary:
$ $18.00 per hour


Schedule:

  • Monday to Friday 9 to 5:30pm
