SUMMARY:

DotHouse Health is a diverse, multi-disciplinary health care and social services agency in Boston's Dorchester (Field's Corner) Neighborhood.

Our state-of-the art facilities provide comprehensive clinical services such as Primary Care, Eye and Dental Care, Radiology/Mammography, Behavioral Health and Laboratory services.

Dot House Health provides over 120,000 visits a year to Adult, Pediatrics and Family Medicine patients.

We are currently seeking a motivated individual to work as a nutritionist within our primary care clinics.

Working closely with providers, this position will include both individual and group nutrition education, tailored to a culturally diverse clientele.

The responsibilities of the job can be flexible, allowing for innovation and customization to the vision of the new hire, and may include providing individual and group counseling, assessing nutritional needs, suggesting diet modifications, conducting educational nutrition programs in the community and providing referrals to outside agencies and programs, as needed.

Under the direction of the Medical Director the Nutritionist plans, organizes and provides nutrition services.


GENERAL DUTIES & RESPONSIBILITIES:


Clinical

  • Works with colleagues and supervisors in defining and establishing appropriate standards, policies and criteria for nutrition services.
  • Is comfortable providing nutrition services using different models such as scheduled visits, integration into primary care, group settings and prepared handouts/materials.
  • Conducts educational programs to foster proper nutrition and food education in the community.
  • In visits, obtains patient's diet history and assesses nutritional status, provides counseling in therapeutic and normal nutrition practices, and provides followup and referrals for other services when appropriate.
  • Advises and helps motivate patients using training in motivational techniques to make dietary changes to alleviate or control medical conditions, such as diabetes, hypertension, obesity, hypercholesterolemia and for normal nutrition practices such as prenatal diets, infant and child feeding, and diet for optimal health.
  • Consults with provider colleagues regarding specific patient cases and nutrition care.
  • Documents services in medical record in a timely manner in accordance with health center policy.
  • Organizes, plans and conducts group education classes as appropriate in collaboration with primary care providers/staff on topics such as weight loss, hypertension, cholesterol and diabetes.
  • Obtains or prepares audiovisual aids, posters, brochures, etc. to assist such classes.
  • Refers to food assistance programs and food pantries when appropriate.
  • Ascertains and provides information to improve nutrition practices in the community.
  • Provides nutrition information for medical providers and staff.
  • Obtains, develops and updates nutrition information materials needed for patients and adapts informational materials to cultural and ethnic preferences of patients, ensuring translation of materials into appropriate languages.
  • Provides education for resident trainee programs and/or participates in agency staff meetings and patient conferences as appropriate.

Requirements:


REQUIRED SKILLS & COMPETENCIES:


Education:

B.S. in Nutrition or equivalent


Experience:


  • Registered Dietician.
  • Three years of community experience preferred
  • Other certifications (such as Diabetes Educator) helpful
  • Proficiency in Vietnamese, Spanish, Cape Verdean or Hatian Creole, preferred.
  • Experience with Epic EMR preferred.
