At Home Solutions is looking for fee-for-service Home Care Field RN in the Albany, Schenectady, and Saratoga county areas to provide high-quality patient care in the home.

• Conduct ongoing patient care and assessments.
• Initiates PlanofCare and makes necessary changes as necessary.
• Prepares written documentation and summaries of the patient's status and progress as needed to continue home health services.
• Maintain close communication with the Director of Patient Services and the interdisciplinary team.
• Reporting all issues and concerns to the Director of Patient Services immediately.
• Completes documentation accurately and submits timely to the agency
• Provides the highest quality care in accordance with DOH regulations.
• Complete evaluation tasks, including reviewing medication and vital signs.
• Administer Physicianprescribed medication.
• Dress or redress wounds and assess the healing progress.
• Provide education to patients and families on proper home health care procedures and strategies.

• Graduate of an approved school of nursing
• Current NYS RN license
• Liability insurance is required.
• Excellent communication skills, detailoriented, patient care, and assessment skills
• Able to function independently with minimum supervision.
• Demonstrates strong critical thinking skills.
• Ability to travel to patients' homes.
• Longterm care 1 year preferred.
• Nursing home care 1 year preferred.
• Rehabilitation center 1 year preferred.
