Job Summary
The Social Worker is responsible for performing social work screenings and interventions for patients in the Skilled Nursing Care.Coordinate discharge planning including arranging services ensuring home safety securing community resources.
- Interviews all new residents assigned area within five days of admission obtains pertinent social history completes initial social assessment initiates resident care plan entry within time frames stated department policy procedure manual.
- Completes timely comprehensive progress notes quarterly assessments specific residents' social service needs problems within time frames stated department policy procedure manual.
- Attends care planning meetings assigned residents provides psychosocial information team members.
