Job Summary
The Social Worker is responsible for performing social work screenings and interventions for patients in the Skilled Nursing Care. Coordinate discharge planning, including arranging services, ensuring home safety,and securing community resources.

and direct the operation of the Social Services Department in accordance with current federal,state and local standards,
