- Completes relapse prevention plan with patients who are in remission. Documents patient progress and treatment recommendations in the registry so they can be easily shared with PCPs (Primary Care Physicians) , psychiatric consultants, and other treatment providers . Monitors patients in person or by phone for changes in clinical symptoms, treatment side effects, or complications .
- Facilitates patient engagement and follow-up care. Tracks patient follow-up and clinical outcomes. Documents encounter in a registry and use the system to identify and re-engage patients. Facilitates referrals for clinically indicated services outside the primary care clinic such as social services, housing assistance , vocational rehabilitation, mental health specialty care, or substance abuse treatment. Facilitates treatment plan changes for patients who are not improving as expected in consultation with PCP and psychiatric consultant. This may include changes in medications or psychosocial treatments with appropriate referrals for additional services.
- Participates in regularly scheduled caseload consultation with the psychiatric consultant and communicates resulting treatment recommendations to the patient's primary care provider. Consultations will focus on patients new to treatment or those who are not improving as expected. Supports and closely coordinates mental health care with the patient's primary care provider and, when appropriate , other mental health providers .
- Provides brief interventions using evidence-based techniques such as Behavioral Activation, Problem-Solving treatment, m otivational interviewing, or other treatments appropriate fo r primary care settings. Screens and assesses patients for common mental health and substance abuse disorders. Provide s basic education and available treatment options. Provide s referrals to evidence-based psychosocial treatments as clinically indicate d .
- Master's degree in psychology , Social Work, or related field .
- BSN or LPN (Licensed Practical Nurse) with mental health training/certification may be substituted .
