    Summary of Duties and Responsibilities:

    The Community Health Worker will be responsible for assisting patients with a variety of care coordination activities. Activities will vary based on patient need. Basic functions will include helping patients to navigate the healthcare system, understand and be connected to Variety Care as a patient-centered medical home, and connecting to community resources to help eliminate barriers to care.

    Responsibilities:

    Primary Duties and Responsibilities:

    1. Applies self proactively and conscientiously to community Health Worker training program(s) as they become available.
    2. Creates a positive experience and relationship with the patients assigned to their panel.
    3. Works directly with the patient in the community, meeting at agreed upon locations or at patients homes as situation and safety allow.
    4. Keeps the patient(s) out of the hospital by supporting regular visits to their primary care medical provider, including assisting patients in learning how to use telehealth.
    5. Keeps the patient(s) actively engaged with their primary care medical provider.
    6. Supports the patient(s) to ensure pick-up of their prescriptions.
    7. Engages the patient(s) to manage their own care.
    8. Supports transitions of care for all patients on the panel.
    9. Helps to keep the patients compliant with their care plans.
    10. Maintains knowledge of and continually learns more about the community cultures and values of the patients on their panel.
    11. Acts as a patient advocate and liaison between the patient and the clinical staff (entire care team) as well as other community service agencies.
    12. Communicates with patients, families, and providers to keep the entire team focused on meeting patient centered care goals.
    13. Communicates with a specified number of patients (the CHWs panel) on a scheduled basis both in person (clinic and community) and via phone.
    14. Works within their scope of work by referring patients to appropriate clinic resources as necessary (e.g., nursing, social services, behavioral health, dental, optical).
    15. Utilizes EMR to inform providers of patient care plan and goals.
    16. Works with team leaders and care managers to identify and reach out to patients needing help.
    17. Demonstrates a patient-centered attitude, supporting the Variety Care effort to encourage patients/parents to become more involved in their own care.
    18. Participates in monthly departmental meetings and team huddles.
    19. Supports Variety Cares pursuit of accreditation as a Patient Centered Medical Home and our commitment to provide care to all Variety patients that is Safe, Effective, Patient Centered, Timely, Efficient and Equitable.
    20. Supports Variety Cares transition to value-based contracting, helping patients receive better care at lower costs by working effectively as part of the care coordination and care management unit.
    21. Embodies the strength of personal character. Places value on being an open and honest communicator who displays high moral and ethical conduct, integrity, adaptability, and sound judgment. Must be a leader in the department and community. Result-oriented problem solver who is responsible and accountable.
    22. Performs other duties as assigned.
    Qualifications:

    Requirements, Special Skills or Knowledge:

    1. High school diploma or GED.
    2. Bi-lingual (English/Spanish).
    3. Possess a positive work attitude and experience working as a team member.
    4. Experience carrying out responsibilities with minimal supervision.
    5. Experience adapting to the changing health care delivery environment.
    6. Ability to travel 60-80% of the time within the Oklahoma City Metro area.
    7. Possess a valid drivers license and reliable vehicle.

    Preferred Requirements, Special Skills or Knowledge:

    1. Two years experience assisting clients with referrals and/or social services.
    2. Job experience in Oklahoma City.
    3. Intermediate Computer skills.

    ADA Requirements:

    1. Must be able to lift 40 pounds and use a dolly.
    2. Able to drive between and to all Variety Care sites upon notice, and willingness to work at multiple locations, as needed.
