    Description

    Who we need:

    This position provides case management services to youth in the CERTAIN program and facilitates Success Workshops (an series of psychoeducational group sessions providing skills training for academic success) in schools. Case management responsibilities will include linking the youth and their family to appropriate agency and/or community services; assist in assessing/service planning/service coordination/referral/aftercare services; assist the youth and their families through the recommended services identified for the youth and their family.

    Hours: 35 hours Monday Friday

    Primary Duties / Responsibilities:

    • CERTAIN Case Manager will facilitate Success Workshops in Schools
    • Provides ongoing case management services to youth and families referred to the CERTAIN program.
    • Collaborate with school personnel to identify youth's needs and to identify resources available for youth and their families at individual school sites and in the community.
    • Provide home visits to youth and their families to assess barriers to school attendance and case management needs.
    • Monitor school attendance and compliance with the recommendations made at Truancy court.
    • Assesses referral needs and assists in making referrals to identified services/agencies.
    • Maintains complete, accurate and comprehensive records updated with information recorded in a timely manner.
    • Assists in attending Case Staffing Committee meetings as needed.
    • Attending appropriate Truancy Court hearings.
    • Assists CERTAIN counselor in securing information contributing to case staffing recommendations and follow-up/aftercare to ensure services are being accessed.
    • Appears for court hearings on CINS Petitions as needed.
    • Services are provided in the office, community, and in home.
    • Participate in various community meetings in which truancy issues are discussed.
    • Other duties as assigned

    Skills, Abilities and Other Requirements:

    • Must demonstrate awareness and acceptance of cultural differences of clients, staff, and community contacts.
    • Must have the ability to develop and maintain effective working relationships with individuals from a variety of cultural and ethnic backgrounds, including clients, staff, professional and community persons.
    • Must demonstrate awareness and acceptance of clients related to sexual orientation and gender identity issues.
    • Must possess excellent organizational skills, well developed verbal and written communication skills, and the ability to effectively deal with complex issues.
    • Must maintain a high degree of professionalism and reliability within areas of responsibility.
    • Should possess characteristics that show sound judgment in fulfilling position duties and responsibilities as well as a cooperative attitude in working relationships with senior members of management.
    • Must have valid Florida Driver's License and auto insurance for personal vehicle and a clean driving history.

    Education and Experience:

    • Bachelor's degree from an accredited university with emphasis in areas of psychology, social work or related field.
    • A minimum of two years' experience working with youth and their families.
    • Must be bilingual (English/Spanish).
    • Must be able to speak, read and write English and Spanish fluently.
    • Knowledge ofDJJ/CINS system, DCF system and available resources in the community, a plus.
    • Demonstrated capacity to provide case management services.
    • Ability to develop and maintain effective working relationships with individuals from a variety of cultural and ethnic backgrounds.
    • Must demonstrate awareness and acceptance of cultural differences of clients, staff and community contacts.
    • Must demonstrate awareness and acceptance of clients related to sexual orientation and gender identity issues.
    • Possess a personal cellular phone for use in accordance with agency policy and procedures.
    • Must have a clear driving record, valid driver's license, and current automobile insurance.
    • Background compliant with requirements of the Department of Juvenile Justice.
    • Valid Florida's driver's license, clear driving record and current automobile insurance.

    Physical Requirements:

    • The ability to lift 10-15 pounds regularly
    • Frequent sitting, standing and/or walking.
    • Frequent use of hands and fingers
    • The ability to hear and respond quickly to sounds.
    • Close, distance and peripheral vision
    • Frequent driving
