- Participates in patient treatment planning and continuum of care for patients with co-occurring and/or diagnosed substance use disorders.
- Conducts or co-facilitates group and/or family sessions related to substance use disorder and/or chemical dependency, as scheduled and documents as required.
- Conducts initial assessments related to substance use disorder and/or chemical dependency, as well as follow up assessments for patients.
- Prepares individual treatment plan for each assigned patient related to substance use and/or diagnosed substance use disorders
- Ensures all documentation regarding patient care, treatment and incidents is completed timely and in a clear, concise manner.
- Acts as a liaison between referral sources and patients.
- Provides crisis intervention to patients, as needed.
- Plans for aftercare for assigned patients.
- Provide case management duties for patients, ensuring individualized quality care.
- Evaluates the need of the patient and determines if referrals to other programs or facilities is needed.
- Provides patient and family education relative to substance use disorders.
- Conducts educational workshops both within the facility and in the community as required.
- Serves as a liaison with self-help groups.
- Conducts ongoing assessments of chemical dependence services required by the Hospital treatment programs.
- Assists in the training of new clinical staff and interns, providing direction and education related to patients with substance use disorders.
- Coordinates the activities of other clinical and direct patient care staff, providing direction and clinical guidance within the scope of practice of Licensed Chemical Dependency Counselors.
- Bachelor's degree in social or health services field from an accredited college or university required. Master's degree in the same field preferred.
- One or more years' experience in counseling field required.
- Licensure obtained through the State Board for Alcohol and Drug Counselors in which the facility operates.
- CPR and de-escalation and restraint certification required (training available upon hire and offered by facility).
- First aid may be required based on state or facility.
