Position Purpose:
Be responsible for overseeing the quality management program of Families Together Community Health Center (FTOC).

Ensure that the clinic meets all quality regulatory standards set by governing/regulatory bodies and patient care outcomes are actively audited and improved upon.


Core Duties and responsibilities, include but are not limited to:

  • Develop and implement a comprehensive quality management program for Families Together Community Health Center (FTOC).
  • Monitor and assess the FTOC's compliance with quality and regulatory standards.
  • Identify opportunities for improvement and develop action plans to address them.
  • Ensure that all clinical staff receive ongoing training on quality management and improvement.
  • Conduct regular audits and assessments of clinical processes and outcomes.
  • Collaborate with the FTOC's leadership team to ensure that quality metrics are met and that patients receive highquality care.
  • Oversee Population Value Based Care grant and all subcomponents.
  • Oversee and support the care coordinator team along with their team deliverables.
  • Support and oversee the FTOC becoming Patient Centered Medical Home (PCMH) certified.
  • Serve as the primary point of contact for regulatory agencies and accreditation bodies.
  • Ensure that the FTOC's policies and procedures comply with all relevant regulations and standards.
  • Provide training to internal stakeholders, specifically employees, to encourage policies and procedures are followed accordingly.
  • Monitor patient satisfaction and implement strategies to improve patient experiences and outcomes.
  • Maintain accurate and detailed records of all quality patient management healthcare gaps, activities and outcomes. (i.e. UDS & HEDIS)
  • Develop and maintain relationships with external quality improvement organizations and experts.
  • Lead and facilitate quality improvement projects and initiatives.
  • Ensure that the clinic's electronic health record system supports quality measurement and improvement.
  • Collaborate with the clinic's risk management team to identify and mitigate potential patient safety risks.
  • Analyze quality data and develop reports to communicate findings and trends to clinical staff, leadership, and other stakeholders.
  • Participate in interdisciplinary committees and teams to promote quality and patient safety.
  • Ensure that the clinic's quality management program is aligned with the organization's strategic goals and priorities.
  • Lead monthly quality board committee meetings to present meaningful summarized quality metrics and develop subsequent action plans.
  • Coordinate a quality workgroup to address quality shortcomings & improvement projects across FTOC.
  • Stay uptodate with developments in healthcare quality management, regulations, and best practices, and communicate these to clinical staff and leadership.
  • Promote a culture of continuous quality improvement and patientcentered care throughout the clinic.
  • Perform other duties as assigned by the Director of Quality & Risk Management.

Educational & Experience Minimum Requirements:


  • Bachelors degree in healthcare management or a related field.
  • Minimum of 5 years of experience in healthcare quality management.
  • Knowledge of quality management principles, tools, and methodologies.
  • Familiarity with regulatory requirements and accreditation standards for federally qualified health centers specifically.**:
  • Excellent communication and interpersonal skills.
  • Strong analytical and problemsolving abilities.
  • Ability to work collaboratively with a diverse team of clinical and administrative staff.
  • Proficient in Microsoft Office and quality management software tools
