Primary PartnerCare is a group of over 40 independent primary care physicians across Suffolk, Nassau and Queens Counties.
Our physicians provide extraordinary care to their adult patients through their participation in CMS Medicare Shared Savings Program and Chronic Care Management Program.
We are seeking a motivated, compassionate, clinically strong RN Care Manager to manage and advocate for our patients post ER, admission, and rehabilitation. You will be a highly visible member of the clinical team, working with the physicians to follow their patients, and advocate for the best and most appropriate care.
Transitions of care for our Medicare patients are fragmented, and quite frankly confusing and frightening to elderly patients and their families.
This position will solve that problem by communicating with the primary care physician, the patient, their loved ones, and the institution to ensure a strong and appropriate plan that maximally benefits the patient's recovery and quality of life is developed and implemented.
Qualifications:
- Valid and unrestricted NYS RN license
- Minimum 10 years clinical nursing experience in an area such as medsurg, critical care, home health
- Minimum two years experience in case management, population health, or discharge planning
- Compassion for the plight of patients' difficulty navigating the healthcare system, and the desire to advocate and make a difference
Salary:
$98,000.00 to $105,000.00 /year + Performance Bonus
Pay:
$98, $106,000.00 per year

License/Certification:
- NYS RN License without restriction (required)
