Requires a nursing license.A professional individual contributor role that may direct the work of otherlower levelprofessionals or manage processes and programs.The majority oftime is spent overseeing the design, implementation or delivery of processes, programs and policies using specialized knowledge and skills typicallyacquiredthrough advanced education.A senior level role that requires advanced knowledge of job area typically obtained through advanced education and work experience.Typicallyresponsible for: managing projects / processes, working independently with limited supervision,coachingand reviewing the work oflower levelprofessionals, resolving difficult and sometimes complex problems.
Job Overview
Theposition,utilizingthe case management process, will work closely with the child/family, the primary Care physician, and all members of the health care team to ensure that a specific population of patients receives comprehensive and coordinated through the continuum. Thepositionconducts comprehensive clinical assessment, develops a patient/family centered care plan including goals of care with the entire complex care team. Thepositionengages the patient and family through coaching with the focus of preparing foroptimumhealth outcomes while preventing admissions,readmissionsand adverse events. Thepositionassesses, plans, implements,coordinatesand evaluates the plan of care in partnership with the family and other members of the health care team.
Job Description
Minimum Qualifications:
1.Registered Nurse (RN) license
2.Three (3) years of experience in care management, ambulatory nursing, community nursing, home care, acute, or subacute setting.
Preferred Qualifications:
1.Bachelor of Science in Nursing (BSN)
2. Specialtynursing certifications
3.Certified Case Manager (CCM)
Duties and Responsibilities:Theduties and responsibilities listedbeloware intended to describe the general nature of workandare not intended to be an all-inclusive list.Other dutiesand responsibilitiesmay be assigned.
1.Preparesfor patient interaction by gathering information from reviewing the patient's medical record when available and communicating with the PCP and other members of the health care team. Completes assessments upon enrollment, transition in care and changes in health status.
2.Coordinate all aspects of care for patients through continuum.
3.Assess the child/family to obtain information to identify and to prioritize, in partnership with the families, the childs health conditions and establish goals that are patient specific.This includesself -management goals.
4.Communicates abnormal findings and concerns in a timely manner to the provider/other members of the care team.Complete a clinical summary to provider.
5.Conductsmedication reconciliation with every transition of care and provides education and consultation with Pharmacist.
6.Developsa care plan with the patient, family;providingall information to the PCP and the health care team.
7.Conductshome assessments to address health care needs especially home safety needs.
8.Documentsthe assessments,interactionsand the care plans within the EMR. Adheres to documentation standards.
9.Utilizesidentified teaching materials and evidence based best practice for teaching patients/families/community..
10.Undertakes quality improvement activities to improve the care management program.
11.Participatesin team meetings;providesupdates on program status and toidentifychallenges and opportunities.
12.Maintainscollaborative, team relationship working with TMIN staff and all affiliated agencies.Works with Medicare and commercial population
13.Possessesand applies the skills and knowledge necessary to plan for and provide care to patients and their families throughout the life span, with consideration of developmental age as well as chronological age. Ability to work effectively with children and families that reflect the full diversity of family structures, cultural patterns, spiritualbeliefsand communities. Thisincludes:Knowledge of growth and development;Ability to interpret age specific data and response to care; andProvideage specific communication.
Physical Requirements:
1.Ambulatory work environment along with inpatient,communityand home care / patient care areas.
2.Occasional exposure to hazardous chemicals/sick patients/bodily substances, extreme noise, radiation.
3.Workrequiresoccasional/frequent/continuous typing/computer keying, lifting/assisting/positioning patients, patient treatment instruments/equipment.
