RN UAS Assessor (Brooklyn, NY)
Do you thrive in home care and want a schedule with work-from-home options? We're seeking a compassionate RN to join our team as a UAS Assessor
This hybrid role offers 2 days working from home and 3 days visiting patients in the Bronx, Manhattan, and Brooklyn areas.
Make a Difference:
- Conduct inhome UAS assessments to evaluate patients' needs.
- Collaborate with the care team to create personalized plans.
- Provide education and support to patients in their homes.
Qualifications:
- NY State RN License (min. 2 years experience).
- 2+ years in home health, home care, or longterm care.
- Strong assessment and communication skills.
- Compassionate and patientcentered focus.
- Computer documentation proficiency.
- Reliable transportation (onsite visits).
