Job Summary:


As a function of the Waupaca County Department of Human Services, the case manager uses a wrap-around approach to support children and families using a strength based, family centered, and Trauma Informed Care model.

This program is designed to provide youth a voice and opportunities that allow them to thrive safely in the community.


This position works with youth that have significant needs (mental health services, special education, youth justice, child protection services, and/or drug and alcohol treatment); youth may be in need of placement, additional supports, and interventions to ensure they may remain safely within their families and communities.

This position works under the direct supervision of the Family & Community Services Manager.


Essential Responsibilities:

Conducts intake assessments, provides case management, and coordinates intensive services for families.

Creates and facilitates child-centered wrap-around teams.

Conducts family-centered, strength-based assessments that concentrate on the child and family strengths and need for support.

Meets regularly with community partners.

Advocates for clients and their families.

Provides services based on Trauma Informed Care principles.

Attends staff meetings and trainings as directed.

Complete and comply with all documentation and program billing requirements.


Carry out the mission and vision of DHS; "We help the people of Waupaca County to be safe and connected".


PERIPHERAL RESPONSIBILITIES:

Performs other duties as assigned by the Family Community Services Manager.


Qualifications:


EDUCATION:
Graduation from an accredited college with a degree in social work or human service field.


EXPERIENCE:
Experience working with the assigned population in need of available service.

Knowledge of the local service delivery system, the target group's needs, the need for integrated services, and the resources available or needing to be developed.


SPECIAL REQUIREMENTS:


Possess a valid Wisconsin Social Work Certification, or be eligible for a Temporary Certificate and obtain that certificate within 6 months of hire and must receive full certification prior to two years of employment with Waupaca County.


Knowledge, Skills & Abilities:

Knowledge base and expertise on the effects of trauma on children and families.

Knowledge base and expertise on child abuse and delinquency and children and families.

Knowledge of children's mental health issues.

Ability to maintain accurate and detailed records.

Ability to follow written and oral instructions.

Ability to communicate effectively orally and in writing.

Ability to establish and maintain effective working relationships.

Ability to maintain agency confidentiality.

Must have available transportation to perform on-site responsibilities.
