    The Clinical Care Coordinator- Certified is the key liaison between the patient and their Patient-Centered Medical Home team. Team includes providers, clinic staff, and other health care professionals. Clinical Care Coordinator works to ensure safety, best practices, and high quality standards are maintained for the patient across the health care continuum.

    Required Qualifications:

  • Completion of an accredited Medical Assistant program.
  • Upon hire: National Medical Assistant Certification
  • Upon hire: National Provider BLS - American Heart Association
  • Upon hire: Obtain appropriate certification(s) within designated timeframe for a particular field of practice (for Specialty areas)
  • Within 1 year of hire: Audiometric, spirometry and EBT (evidential breath alcohol) testing to possess or gain the appropriate certifications (for Occ Health)
  • 3 years Recent health care experience working as a Medical Assistant.
  • 6 months Specialty areas: Recent experience working as a Medical Assistant.
  • Preferred Qualifications:

  • College.
  • Experience with electronic medical records and/or computerized billing systems.
